Fee Schedule

• New Patient Evaluation: $750
• Follow Up Visit: $200
• Extended Follow Up Visit: $400
• All other work is billed at $400 per hour, or $65 per 10 minute increments of physician time (this is rare)
  • We do not charge for quick work that takes < 10 minutes of the doctor’s time (examples: signatures on filled out forms, quick email replies, quick phone calls, etc)
  • We will charge for work that takes a more extensive amount of time (lengthy conversations by email or voice outside of appointments, extensive forms/letters, genesight test analysis, etc), or any other work that requires > 10 minutes of physician time.
• Legal and Court-Related Services: $5000/day (or any portion thereof), plus $500/hour inclusive of any preparation time, travel time, and any other expenses.  Dr. Heck does not typically do court/legal evaluations or testimony.
• Bounced Check: $75 fee
